Dear Ms. DiRienzo:
This letter requests Amendment No. 1 of the Massachusetts Department of Environmental Protection
WPA Form 5 — Order of Conditions for our property located at 11 North Cove Landing, South Yarmouth,
MA 02664. We would appreciate this matter being on the Agenda for the Conservation Commission for
their next scheduled meeting on July 20, 2023 at 5:00 PM.
The supporting materials for Amendment No. 1 include the following:
Construction Documents: Project No. 14093, prepared by Annino Incorporated, 125 North
Washington Street, North Attleboro, MA entitled Rodgers Addition, for 11 North Cove Landing,
Yarmouth, MA revised as of February 8, 2019;
Ownership of the home is updated to reflect the joint ownership of Josephine White Rodgers,
Ph.D. and Kenneth W. Rodgers, Ed.D.; and
Specifically, the Construction Document is relevant to the following items:
a. Deck on the west side of home;
b. Outdoor shower on the northeast corner of the home; and
c. Concrete pad at the base of the steps from west side of the home from the new deck.
The materials for the pad that is stated as concrete will be amended to conform to the
Conservation Commission's preference that this pad be made of pervious materials, such
as pavers or stepping stones in which the pavers/stones are dried laid and not in mortar.
We including the following documents with Amendment No. 1 as specified by the Conservation
Commission:
1. 8 copies of the updated plan, titled as Construction Documents: Project No. 14093
2. A pdf of the original letter, Construction Documents and other supporting materials have been
emailed to: bdirienzo@yarmouth.ma.us.
3. Abutters List certified by the Town Assessor's Office, along with Certified Mail Recipts (PS Form
3800) for all abutters.
4. Legal ad fee of $10 will be paid by check with the submission of this letter to the Conversation
Commission.
All of the requested materials are either attached to this letter or separately filled with the Conservation
Commission office on or before July 5, 2023 in preparation for the Conservation Commission meeting on
July 20, 2023.
Your assistance in the preparation of this Letter of Amendment and all of the attachments has been most
appreciated.
