Dear Mr. Gardner,
As per your request and in accordance with 310 CMR 15.021(3) of the State
Environmental Code-Title 5, an as-built review of the septic system at the subject site
has been conducted. I hereby certify that the Sanitary Subsurface Disposal System
has been installed in substantial compliance with the design plan by Engineering
Works, Inc., dated 11/28/22. Driplines were raised from elevation 6.7 to elevation 7.6
due to rise in water table encountered during installation. The septic system consists of
a new 1500 gallon septic tank, pump chamber and Perc-Rite Drip Dispersal system..
