Dear Mr. Gardiner:
On behalf of our clients, Lisa and James Valone, on June 14, 2022, my office inspected the installation of
their new guest house and septic tank, and found the installation to be in substantial compliance with the
approved plan entitled Proposed Guest House/Garage Addition Plan, revised 9/21/2022, and in
compliance with the provisions of 310 CMR 15.00.
An H-20 rated 1500 gallon septic tank was installed as shown on plan for remodeled guest house, with
new 4" PVC sewer piping in and out of tank, and with 4" PVC tees on inlet and outlet pipes. Gas baffle
was also installed.
24" cast iron frames and covers will be set at finish grade over inlet and outlet tees.
After inspection,contractor was consulted about verifying invert elevation into distribution box. The
invert was verified and slope from new septic tank to d-box was adequate. Contractor also installed
concrete risers to bring d-box cover to grade. (Photo 1).
The as-built sketch by the contractor of the new septic tank and existing d-box is attached here.
