As the designer of the system for the referenced site,it was determined upon installation that the
system was installed per the requirements and allowances of the State Title V and the Yarmouth
Board of Health Regulations. All risers,and septic tank tees where observed installed at the time
of inspection. "The issuance of this certification shall not be construed as a guarantee that the
system will function as designed.
