RE: 911 Great Island Road, West Yarmouth; SE 83-1852- Certificate of Compliance
To the Conservation Commission,
Attached please find Form 8A and a check in the amount of $75 for a Request for
Certificate of Compliance for SE 83-1852 issued in 2009 for coastal stabilization at
911 Great Island Road, West Yarmouth.
A series of efforts were made during the term of the Order for maintaining sand cover.
Several storms occurring in 2018 eradicated the southern segment of coir logs. The north
segment, north of the stairs remains in place.
The applicant intends to file a Notice of Intent for purposes of continuing with sand
cover. However, an alternate approach would be some form of on -going, perhaps time
limited, provision for sand nourishment in the COC. That option is requested with this
application but is at the discretion of the Commission.
Based on my knowledge, information, belief and with professional judgement from a
long-term monitoring of the site and related permit plans, the completed project warrants
issuance of a Certificate of Compliance.

B. Submittal Requirements
Requests for Certificates of Compliance should be directed to the issuing authority that issued the final
Order of Conditions (OOC). If the project received an OOC from the Conservation Commission, submit
this request to that Commission. If the project was issued a Superseding Order of Conditions or was the
subject of an Adjudicatory Hearing Final Decision, submit this request to the appropriate DEP Regional
