TO: Town of Yarmouth Health Department

SUBJECT: 167 Station Ave, South Yarmouth, MA Septic System
Per the requirements of the Town of Yarmouth Health Department,
this office determined that the installation of the septic system as
designed by this office was installed in compliance as allowed
within the Yarmouth Health Department Regulations and the
requirements of State Environmental Code Title V as approved.
This certification of installation shall not be construed as a
guarantee that the system will operate correctly into the future due
to uncontrolled use of the system which is outside of the design
and installation parameters of the system.
