Dear Mr. Gardiner,
As per your request and in accordance with 310 CMR 15.021(3) of the State
Environmental Code-Title 5, an as-built review of the septic system at the subject site
has been conducted. I hereby certify that the Sanitary Subsurface Disposal System
has been installed in substantial compliance with the design plan by Engineering
Works, Inc., dated 4/5/23. The septic system consists of an existing 1000 gallon septic
tank, new distribution box and 6-LC6 precast H-20 chambers surrounded with 3' of
stone.
