Yarmouth Assessor’s locator map highlighting parcel(s) where work is proposed, including map and parcel numbers 1 original and 7 photocopies of the plan/sketch, folded separately, right side out with title and address visible. All plans shall reference NAVD1988 unless otherwise noted. Landscape plans shall be detailed to show proposed and existing conditions, native species, size and spacing. Work Description (use additional paper and/or provide plan(s) of work, if necessary): The proposed project entails the raze and repalcement of a single-fmaily dwelling at 10 and 12 Braddock Street. A proposed shed will be installed in the northeastern corner of the Site. The project also proposes the installation of a Tittle V septic tank, distribution box, and a leaching field. The existing septic system will be pumped, crushed and disposed of in accordance with standard practice. Flood vents and roof drains are also proposed to control stormwater. Identify provisions of the Wetlands Protection Act or regulations which may exempt the applic ant from having to file a Notice of Intent for all or part of the described work (use additional paper, if necessary). All components of the proposed project are located only within FEMA Floodzone AE. Both lots are cleared and/or developed and no significant removal of vegetation is proposed. The proposed dwelling will improve stormwater collection and management on the site and will not alter stormwater flow towards the adjacent sites. Addition of impervious area and disturbance has been limited to the greatest extent practicable. Therefore, no significant impact the jurisdictional resource areas or adjacent properties is anticipated. 3. a. Request for Determination of Applicability 10-12 Braddock Street South Yarmouth, MA PROJECT DESCRIPTION 1.0 INTRODUCTION The proposed project located at 10-12 Braddock Street (the Site), spanning two parcels, in South Yarmouth, MA involves the raze and replacement of a single-family dwelling and a new Title V septic system This application is submitted in accordance with the Massachusetts Wetlands Protection Act and the Town of Yarmouth Wetlands Protection By-law. 2.0 SITE DESCRIPTION The area where work is proposed is a single-family residential property located at 10-12 Braddock Street in South Yarmouth. Neighboring properties and Forested land exist to the east, west, north, and south. The wetland resource areas and areas of conservation jurisdiction on the site that are protected under the Wetlands Protection Act and the Town of Yarmouth's Wetland Protection Bylaw include the following: • Land Subject to Coastal Storm Flowage, Zone AE (Elevation 11) 3.0 PROPOSED ACTIVITIES The purpose of this application is to authorize the following activities: The proposed project involves the raze and replacement of a single-family dwelling, as well as a Title V septic system. The dimensions of the new dwelling will be approximately 56.5’ by 30.25’, overlapping a portion of the existing dwelling which is located on parcel at 12 Braddock St. 7 flood vents and 5 roof drains will be installed to control stormwater. A 10’ by 14’ shed is also proposed to be installed in the northeastern corner of the Site. The project also proposes the installation of a new 1500-gallon septic tank, distribution box, and a 41' by 11' leaching field. The existing septic system in the northwestern corner of the Site will be pumped, crushed and disposed of in accordance with typical septic system retirement procedures. All components of the proposed project are located only within FEMA Floodzone AE. Both lots are cleared and/or developed and no significant removal of vegetation is proposed. The proposed dwelling will improve stormwater management on the site and will not alter stormwater flow towards the adjacent sites. The addition of impervious area and disturbance has been limited to the greatest extent practicable. Therefore, no significant impact the jurisdictional resource areas or adjacent properties is anticipated. 4.0 MITIGATION Temporary compost silt sock will be used as a sediment control and will also act as a limit of work. Following construction, all disturbed areas will be loamed and seeded and returned to original grade and/or planted with mitigation planting.
