Dear Mr. Gardiner,
As per your request and in accordance with 310 CMR 15.021(3) of the State
Environmental Code-Title 5, an as-built review of the septic system at the subject site
has been conducted. I hereby certify that the Sanitary Subsurface Disposal System
has been installed in substantial compliance with the design plan by Engineering
Works, Inc., dated 5/17/23. The septic system consists of an existing 1000 fiberglass
gallon septic tank, new distribution box and 3 rows of 3, Cultec C-4 HD Field Drain
units with 3/4" stone added over the top of the units for structural stability and air
movement. Filter fabric was placed over the stone. These changes were approved by
me.
