On October 22, 1981, the Commission issued an Order of Conditions for the above -referenced site, for the construction of a timber pier addition. A site visit/inspection/asbuilt were made for the purpose of closing out the Order of Conditions. The 8' by 64' extension that was approved under this Order was built in accordance with the plan of record. This is to certify, to the best of my knowledge and belief, that the work was performed in substantial compliance with the approved plan and Order of Conditions.
