All applications must be submitted two weeks prior to the next scheduled Conservation
Commission meeting. Refer to Meeting Schedule.
Failure to follow the Administrative Checklist shall result in an Administrative Incomplete
Application and will not be scheduled for a Public Hearing.
Contact the Conservation Office if you need assistance.

B. Submittal Requirements
Requests for Certificates of Compliance should be directed to the issuing authority that issued the final
Order of Conditions (OOC). If the project received an OOC from the Conservation Commission, submit
this request to that Commission. If the project was issued a Superseding Order of Conditions or was the
subject of an Adjudicatory Hearing Final Decision, submit this request to the appropriate DEP Regional

On September 24, 2003, the Yarmouth Conservation Commission issued
an Order of Conditions for the above -referenced site, for the
proposed float addition to an existing pier, ramp and float
facility.
A site visit/inspection and as -built were performed in July of
2023 for the purpose of closing out this Order.
The following was noted:
• The southerly float configuration is shorter than what was
permitted
• The decking is shaped slightly differently
• Steps to the beach off the bulkhead were added
• It is unknown if 500 quahogs were planted
This is to certify, to the best of my knowledge and belief and
with the minor exceptions listed above, that the pier, ramps and
floats are in substantial compliance with the approved plan and
Order of Conditions. We respectively request the issuance of a
Certificate of Compliance to close out SE83-1647.
