OFFICE
All filings must be made on Town of Yarmouth forms.
All applications must be submitted two weeks prior to the next scheduled Conservation
Commission meeting. Refer to Meeting Schedule.
Failure to follow the Administrative Checklist shall result in an Administrative Incomplete
Application and will not be scheduled for a Public Hearing.
Contact the Conservation Office if you need assistance.

On 3/23/95, the Commission issued an Order of Conditions for the
above -referenced site, for the addition of fill on a Coastal Bank,
within Land Subject to Coastal Storm Flowage.
A site visit/inspection/asbuilt were made for the purpose of
closing out the Order of Conditions. Approximately 50 to 55 yds
of fill had been added to the rear of the dwelling, providing a
more level back yard. The grass (not sod or irrigated) was
extended. The area is completely stable, with a thick buffer of
vegetation between the lawn and wetland. During the inspection an
old concrete wall was discovered near the wetland, completely
covered in vegetation. This was not seen during the original
survey and so was not shown on the record plan. Sometime between
1995 and recent times, a paver driveway (open joints) was
installed in place of the gravel driveway.
This is to certify, to the best of my knowledge and belief, that
the work performed under this Order was completed in substantial
compliance with the approved plan and Order of Conditions, with
small deviations in the final grading. We hereby request the
issuance of a Certificate of Compliance at this time.

B. Submittal Requirements
Requests for Certificates of Compliance should be directed to the issuing authority that issued the final
Order of Conditions (OOC). If the project received an OOC from the Conservation Commission, submit
this request to that Commission. If the project was issued a Superseding Order of Conditions or was the
subject of an Adjudicatory Hearing Final Decision, submit this request to the appropriate DEP Regional
