1, David D. Flaherty Jr., Registered Sanitarian, duly licensed in the Commonwealth of
Massachusetts, do hereby certify that I have visually inspected the cesspool system abandonment
and certify that the cesspool system was abandoned within substantial compliance and generally
conforms within acceptable tolerance to the regulations set forth in 310 CMR 15.000 (Title 5)
and the Town of Yarmouth Board of Health regulations.
The (2)existing cesspools, while not connected to the existing septic system, were dry and filled
with clean medium sand.
Final inspection was on 9/7/2023
