All filings must bep made on Town of Yarmouth forms. The filing deadline is two weeks prior to the next
scheduled Conservation Commission meeting for new applications. Failure to follow this Checklist shall
result in an Administratively Incomplete Application and will not be advertised for a Public Hearing. Refer to
the meeting schedule. Contact the Conservation Office if you need assistance.
Does the property have any outstanding/expired Orders of Conditions? If so, please file a Request
for Certificate of Compliance for each outstanding/expired Order, as the Commission may not issue a
new Order until any outstanding/expired Orders have been closed and recorded. (*see page 2)

1 detailed narrative of the project including: existing &proposed conditions, construction sequence, type
of equipment, staging locations, drainage and stormwater, erosion controls, invasive species
management and alternative analysis. The narrative shall include how the project meets performance
standards per 310 CMR 10.0 & TOY Wetland Bylaw, Chapter 143, BVW delineation sheets or other
resource area calculations, and supporting information.

CMR 10.00and abutter notification form. Abutters list must be certified by the Town Assessor's office.
The Assessor's office requires 7 days advance notice. All abutters must be notified via certified mail.

supplemental information to the Town of Yarmouth Waterways/Shellfish Committee via the Natural
Resources office by certified mail or hand delivery. The applicant or his/her representative must provide
the Conservation office proof that this has been done or the filing will not be accepted.

you need to file with the Yarmouth Board of Appeals? If so, you must file with both boards at the same

NOTICE OF INTENT
#179 River Street, South Yarmouth, MA
The applicant is pleased to submit a Notice of Intent application for the raze and replace of an
existing single-family structure. The 36,384 s.f. property is located in a residential
neighborhood and located within Land Subject to Coastal Storm Flowage (LSCSF) zone AE
(EL 11). The new dwelling will utilize the existing 4-bedroom leaching field that was installed in
2013. A new septic tank is proposed.
No work will take place within Priority/Estimated Habitat of endangered species. All work will
take place outside the 200' riverfront area to the southeast.
Exposed soils will be loamed and seeded within 30 days of completion of the work and final
grading work as weather permits.
Land Subject to Coastal Storm Flowage
The site is affected by 100-year flood elevation AE (EL 11). The proposed dwelling will
conform to floodzone and building code standards.
Approximately 1,200 cubic yards of fill is proposed. Due to the large scale of coastal flooding
events construction and grading activities will have no effect on the 100-year flood elevation.
Roof drywells and rain gardens are proposed to help infiltrate runoff. The existing paved
driveway will be replaced with a gravel drive which will also help reduce runoff.
Compliance with the MA Wetlands Protection Act and Yarmouth Bylaw Chapter 143
Regulations:
The proposed construction activities will not have a deleterious effect upon any wetland values
as listed in the Bylaw. Roof drainage will be mitigated utilizing downspouts to drywells or drip
lines to stone trenches. The new dwelling will be brought up to flood zone compliance and the
presence of the gravel drive and rain gardens will reduce the stormwater runoff. All machinery
will access the areas of proposed construction through the existing driveway.
