I, David D. Flaherty Jr., Registered Sanitarian, duly licensed in the
Commonwealth of Massachusetts, do hereby certify that I have visually inspected the
system installation and certify that the system, as constructed, was installed
within substantial compliance and generally conforms within acceptable tolerance to
the regulations set forth in 310 CMR 15.000 and the Town of Yarmouth Board of Health
regulations.
The following inspections were performed:
1)Septic Tank and Pump Chamber Installation-9/6/2023
2)Bottom of the hole for SAS and Liner Installation-9/8/2023
3) Sand fill and double washed stone with SAS piping-9/25/2023
2)Risers and dbox installation, 4" dbox tee in lieu of 2",Final Inspection-9/29/2023
Such certification shall neither be misconstrued as a guarantee that the system will
operate satisfactorily nor certification of alterations after inspection.
