This license issued by this application is valid for the calendar year January through December provided the type of
entertainment specified above does not change. In the event of a change in the type of entertainment different than
indicated above, a new application will be required, and a new license issued.
Documents Required:
•Detailed Floor Plan of Event (Showing egress, tables, seating, dance floor, food setup, where entertainment will
be set up, emergency lighting, etc.)
•Certificate of Insurance showing Workers Compensation Insurance.
Fees:
•Hearing Fee: $65 (Waive if combined with Annual Alcohol License)
•License Fee: $105
•Legal Ad Fee: To be determined once legal ad is placed.
•Abutter Notice Mailing is paid by the applicant.
