STRUCTURAL NOTES
1. Hold all rafters down to wall plate with Simpson H2.5A Tie. If
toe nailing of rafters to ridge beams result in rafter end splitting or
less than 1" of nail lenght in rafter wood, reinforce joint with joist
hangers.
2.Fasten rafters to ridge beam with Simpson LSSU - series or
Simpson straps over top of sheathing spaced at 16" o.c.

CONSERVATION COMMISSION REQUIREMENTS:
1.) No work of any kind shall occur on the Resource Area side of the work limit line. This
line shall be established prior to any construction or work.
2.) All imported fill shall be clean and not contain any foreign debris such as cement,
bituminous concrete, rubber, metal, trash, boulders, etc.
3.) Debris, including litter, shall be picked up immediately. Any excess material that is
excavated from the job site shall be disposed of at a location approved by the
Conservation Administrator or in a covered dumpster. No debris shall be allowed in a
Wetland or Waterway.
4.) Washing of equipment/tools shall be done OFF -SITE or handled in a manner
appropriate by the Conservation Administrator.
5.) No equipment shall be refueled, nor shall fuel be stored within 100 feet of a resource
area.
6.) Any area that is disturbed during construction shall be revegetated immediately or as
soon after as the growing season commences. Permanent stabilization shall be carried
out in order to minimize erosion.
7.) The proposed sewage disposal system shall meet the applicable Title 5 wetlands
setbacks as set forth by the Yarmouth Health Department.
8.) The "work limit" line shown on this plan shall be established with Erosion Controls (i.e.
straw bales, staked filter fabric, bark mulch, "silt sacks", products made with rice straw,
and other weed -free products or approved by the Yarmouth Conservation Commission).
Hay bales are not allowed. The contractor is responsible to implement and maintain
appropriate and adequate erosion controls measures throughout the construction process.

GENPPA' NOTES
1. UNLESS OTHERWISE NOTED, ALL SYSTEM COMPONENTS AND CONSTRUCTION

CODE AND ANY APPLICABLE LOCAL RULES.

ELEVATION = 8.78' FOR A DISTANCE OF 15' AROUND THE PERIMETER OF THE SAS. UNLESS A
40 MIL GEOMEMBRANE LINER IS PLACE AT LEAST FIVE FEET FROM S.A.S. AND THE TOP OF
THE LINER IS NOT LESS THAN THE BREAKOUT ELEVATION.
5. SLOPE ALL SOLID PIPE AT 1.0% MINIMUM.
6, THIS SYSTEM IS NOT DESIGNED FOR A GARBAGE DISPOSAL.
7. LOCAL BOARD OF HEALTH AND DESIGN ENGINEER TO BE NOTIFIED PRIOR TO BACK
FILLING WHEN SYSTEM IS NEARLY COMPLETE AND READY FOR INSPECTION. SYSTEM IS
NOT TO BE BACK FILLED WITHOUT FIRST OBTAINING APPROVAL FROM BOARD OF HEALTH
AND DESIGN ENGINEER.
8. ELEVATIONS BASED ON NAVD 88 DATUM. SEE BENCHMARK LOCATION AND ELEVATION AS
SHOWN ON PLAN.
9. CONTRACTOR SHALL VERIFY ALL UTILITY LOCATIONS PRIOR TO CONSTRUCTION
THROUGH DIG -SAFE AT LEAST 72 HOURS PRIOR TO COMMENCING WORK ON SITE AT
1-888-DIG-SAFE AND ANY OTHER APPLICABLE AGENCIES. REPORT ANY DISCREPANCIES
TO THE DESIGN ENGINEER.
10. ALL JOINTS WHERE PIPE ENTERS AND EXITS CONC. STRUCTURES SHALL BE MADE WATERTIGHT.
11. NO DETERMINATION HAS BEEN MADE AS TO COMPLIANCE WITH DEEDED OR ZONING
REGULATIONS. OWNER/APPLICANT IS TO OBTAIN SUCH DETERMINATION FROM
f APPROPRIATE AUTHORITY.
12. ALL SEPTIC SYSTEM COMPONENTS SHALL WITHSTAND H-10 LOADING UNLESS LOCATED
UNDER MORE THAN 3 FEET OF COVER OR LOCATED UNDER PAVEMENT, DRIVES, OR
TRAVELED WAYS IN WHICH CASE THEY SHALL WITHSTAND H-20 LOADING.
13. DOUBLE WASHED CRUSHED STONE SHALL BE FREE OF ALL DIRT, DUST AND FINES.
14. WHERE REQUIRED, CONTRACTOR SHALL REMOVE ALL LOAM, SUBSOIL AND UNSUITABLE
MATERIAL IN AREA BENEATH AND FOR 5 FT. ON ALL SIDES OF LEACHING FACILITY.
REPLACE ALL UNSUITABLE MATERIAL WITH CLEAN COARSE SAND FREE FROM CLAY,
FINES OR OTHER UNSUITABLE MATERIAL IN ACCORDANCE WITH 310 CMR 15.255(3).
15. CONTRACTOR SHALL NOTIFY DESIGN ENGINEER OF ANY DISCREPANCIES FOUND IN
SITE CONDITIONS FROM THOSE SHOWN PRIOR TO CONTINUATION OF WORK.
