Serious conditions were found in the property and are listed below. If you will be requesting an extension beyond this date, a hearing in accordance with 105 CMR 410.800 through 410.860 must be conducted. Failure to comply with this order will subject you to a fine of $300.00 per day upon conviction in Housing Court (105 CMR 410.920 through 410.930). You also have the right to request in writing, a hearing before the Board of Health (105 CMR 410.800) within seven days of receipt of this order. You will be contacted with the date of the hearing.
