Dear Mr. Gardiner,
As per your request and in accordance with 310 CMR 15.021(3) of the State
Environmental Code-Title 5, an as-built review of the septic system at the subject site
has been conducted. I hereby certify that the Subsurface Sanitary Disposal System
has been installed in substantial compliance with the design plan by Engineering
Works, Inc., dated 9/25/23, revised 11/2/23. Prior to changes being made, the Health
Department was informed that field conditions were encountered that warranted the
changes. The septic system consists of a new 1500 gallon tank, D-Box, and 2-500
gallon chambers surrounded with 4' of stone.
