Dear Mr. Gardiner,
As per your request and in accordance with 310 CMR 15.021(3) of the State
Environmental Code-Title 5, an as-built review of the septic system at the subject site
has been conducted. I hereby certify that the Subsurface Sanitary Disposal System
has been installed in substantial compliance with the design plan by Engineering
Works, Inc., dated 10/11/23, revised 10/21/23. The septic system consists of an
existing 1500 gallon plastic septic tank, a new distribution box and one row of 5, high
capacity H-20 Infiltrators, surrounded with stone, as shown on the plan.
