The filing deadline is two weeks prior to the next scheduled Conservation Commission meeting for new applications. Failure to follow this Checklist shall result in an Administratively Incomplete Application and will not be advertised for a Public Hearing. Refer to the meeting schedule. Submitting an Application: Hardcopy filing dropped off or mailed to the Conservation Office including: 1 RDA Administrative Checklist 1 complete RDA application WPA form 1 with original signatures (double sided). * Detailed narrative of the project with sufficient information to enable the Commission to determine whether the proposed work will alter an area subject to protection. If BVW or vegetated wetlands are present, delineation sheet(s), coastal bank calculation and other relevant material if applicable (not applicable for flood zone only projects). Yarmouth Assessor’s locator map highlighting parcel(s) where work is proposed, including map and parcel numbers 1 current abutters list identifying the property owners who are to be notified and abutter notification form. All abutters to be notified via certified mail, Certified Mail Receipts (PS Form 3800) must be provided. 1 original and 6 photocopies of the plan/sketch, folded separately, right side out with title and address visible. All plans shall reference NAVD1988 unless otherwise noted. Landscape plans shall be detailed to show proposed and existing conditions, native species, quantity, size and spacing. Activity/Work Description (use additional paper and/or provide plan(s) of Activity, if necessary): The proposed project involves the installation of an anchored concrete pad beneath the existing raised single-family dwelling. Existing gravel layer will be removed. Concrete pad will be limited to the footprint of the raised dwelling. As the proposed pad will be limited to the underside of the raised dwelling, no significant earth disturbance will be required and the concrete pad will be surrounded by drip trenches, the proposed project will not result in adverse impacts to the Buffer Zone to Coastal Bank/Coastal Beach or Land Subject to Coastal Storm Flowage. b. Identify provisions of the Wetlands Protection Act or regulations which may exempt the applicant from having to file a Notice of Intent for all or part of the described work (use additional paper, if necessary). Under 310 CMR 10.02(2)(b)(2), minor activities within Buffer Zone can be approved, provided that the work is conducted in a manner that reduces impacts during/after construction to the greatest extent. 3. a. Request for Determination of Applicability #15 Shore Road West Yarmouth, MA PROJECT DESCRIPTION 1.0 INTRODUCTION The proposed project located at 15 Shore Road (the Site) in West Yarmouth, MA. This application is submitted in accordance with the Massachusetts Wetlands Protection Act and the Town of Yarmouth Wetlands Protection By-law. 2.0 SITE DESCRIPTION The project site consists of a single-family residential property located at 15 Shore Road in West Yarmouth. Neighboring residential properties exist to the north, and east, with coastal beach, coastal bank, and Lewis Bay to the southwest. The wetland resource areas and areas of conservation jurisdiction on the site that are protected under the Wetlands Protection Act and the Town of Yarmouth’s Wetland Protection Bylaw include the following: •Land Under Ocean (LUO) •Coastal Beach •Coastal Bank •Land Subject to Coastal Storm Flowage FEMA Floodzone elevation VE 15 and 13 3.0 PROPOSED ACTIVITIES The purpose of this application is to authorize the following activities: The proposed project involves the replacement of gravel beneath the existing raised dwelling with an anchored, concrete pad. The existing gravel beneath the dwelling will be removed. The concrete pad (roughly 1,054 square feet in size) will be contained to the footprint of the raised dwelling, aside from the stormwater filtration features, including drip trenches along the edges of the dwelling. Only limited earth disturbance will be required for the installation of the concrete pad. Because the concrete pad will be contained to the footprint of the structure (minus the drip trench edges of the dwelling), no increase in impervious area is proposed. No change in grade or alterations to stormwater retention within LSCSF and the 100-foot Buffer Zone to Coastal Bank and Coastal Beach are anticipated. 4.0 MITIGATION The proposed work will be limited to work beneath the existing raised dwelling; therefore, no significant excavation or removal of vegetation is anticipated. Access for construction equipment will be gained from the existing paver stone driveway or existing lawn areas. If any rutting occurs during construction activities, disturbed areas will be returned to their original grade and stabilized with loam and grass seed and or/hay.
