SUBJECT: 232 Blue Roack Road, South Yarmou
The septic system installed at the subject location by Ruberti
Construction LLC was installed per the plan and the allowances
indicating compliance with the State Title V requirements.
The septic design plan as approved by the Yarmouth Health
Department met the requirements of Title V and the Yarmouth
Board of Health Regulatioins. Theinstallation of components and
elevations meets the requirements of the design plan.
The tee in the distribution box and risers were observed during the
inspection. The pump was operated, alarm sounded and the weep
hole draining at the time of inspection.
This certification shall not act as a guarantee that the system will
operate as designed as such is dependent on the type of use and
disposal in the system by the owner.
