Dear Mr. Gardner,
As per your request and in accordance with 310 CMR 15.021(3) of the State
Environmental Code-Title 5, an as-built review of the septic system at the subject site
has been conducted. I hereby certify that the Sanitary Subsurface Disposal System
has been installed in substantial compliance with the design plan by Engineering
Works, Inc. & Richard J. Hood PLS, dated 9/25/23 . The septic system consists of an
existing 1000 gallon septic tank, new D-box and 2-500 gallon precast chambers
surrounded with stone.
