Traffic Assessment: a) Crash Data: i) Please calculate the crash rate for Hockanom Road. The memo that was prepared looked at crash history, traffic generation, and sight distance exclusively. No traffic counts were conducted so it is not possible to calculate the rate, as volumes are necessary. However, given the low number of accidents, you can expect that the rate is well below the state average. ii) Is there a reason the Outward Reach intersection was not analyzed? We have pulled accident data for Outward Reach intersection with 6A (see attached). There were 2 property damage only accidents at that location in the latest 5 years of data that is available. iii) Would the Yarmouth Police Department have any supplemental crash data or is the MassDOT database the most accurate? We typically get police accident data when there is a demonstrated issue (based on MassDOT data). That is not the case here so I don’t believe pulling police records would be necessary. b) Sight Distance: i) In Table 3 – Route 6A at Proposed Sight Driveway Stopping Sight Distance (SSD), it would appear that the measured Stopping Sight Distance traveling eastbound would be the higher number, please confirm. The SSD is 900 + eastbound and 370+ westbound. They are shown backwards in Table 3. Sorry about that. ii) Why is the Intersection Sight Distance different for looking Left and Right? I have never seen this before. Intersection sight distance is based on the view exiting the driveway. Looking left is   Attention!: This email originates outside of the organization. Do not open attachments or click links unless you are  sure this email is from a known sender and you know the content is safe. Call the sender to verify if unsure.  Otherwise delete this email.   2 substantially different than looking right at this location due to the geometry of the roadway, grades, and other features. iii) Are the measured values in Table 3 based on existing conditions, or do they take into consideration the proposed regrading and vegetation removal in the Sight Easement area? If existing conditions, what are the estimated sight distances with the recommended Sight Easement? Are there any other improvements that could be made to increase the ISD? Yes the sight distance referenced in the table is based on existing conditions. The proposed would be better than existing for both the site driveway and potentially Hockanom Rd. The clearing would have to be done and then measurements taken to really understand the improvement given the horizontal features to the east of the roadway but it appears the benefit could be in the 30 to 50 feet range looking east for ISD (critical movement). Can consider widening the sight triangle to the extent practical. iv) The Sight Easement recommended in the VHB Traffic Assessment is larger than that shown on the Plans. Ultimately, the Sight Easement including any additional slope grading needed should be adjusted on the Plans to meet that recommended in the VHB Assessment. John to provide. c) 400’ Waiver: The Applicant is requesting a waiver from the requirement that street intersections on Route 6a be spaced not less than 400’ apart (4.2.1(j)). It would appear that access to the site off Route 6a could be located 400’ from both Hockanom and Outward Reach. Please have VHB provide some documentation, conclusion or recommendation to support the requested waiver (i.e. is the proposed intersection location safer in their professional opinion). The site driveway has been positioned in a place that allows adequate sight distance in both directions. If the driveway were shifted to be 400 feet from both Hockanon and Outward Reach, the SSD westbound, and ISD looking right would be compromised and not conform to minimum standards necessary. Also by aligning with Hockanom Rd we are minizing the conflict points. Having three offset intersection in close proximity to one an other should be avoided when possible. d) MassDOT: The Comment & Resolution Form does not provide any information on whether MassDOT would feel comfortable issuing the highway access permit, it just outlines the procedures you need to follow to submit an application. Were there any substantive comments on the access location made by MassDOT? We have not reached out to MassDOT yet. That is typically done post local approvals. However, given my experience on projects like this, as long as we meet the sight distance requirements (we do) and there aren’t any major safety concerns (there are not), approval of the access driveway is very likely.   2. Homeowners Association: The Homeowner Association document submitted outlines the creation and operation of the Association. It mentions Maintenance Covenants in 4.1 and Declaration of Protective Covenants in 4.7, but based on the context these seem more related to what people do on their individual lots and were not submitted with the application materials. There needs to be some type of Maintenance Covenant that addresses the requirements for operation and maintenance of the private roadways, the utilities, the drainage systems (per an approved O&M Plan), landscaping, Open Space and Sight Easement, along with legal conveyance of the Open Space to the Homeowners Association who would be responsible for payment of taxes. A restriction enforceable by the Town will also need to be recorded to preserve the Open Space per Section 402.6.2.
