To Whom It May Concern:
On several occasions personnel from our office field verified the locations and elevations of the
septic system components installed as per plan titled "Proposed Dwelling & On-site Sewage
Treatment and Disposal System Prepared for: John-Patrick Python & Julia Loughman", dated
March 28, 2022 and revised April 19, 2022.
In accordance with the requirements of 310 CMR 15.021(3) this letter shall serve as certification
that the septic system was installed in compliance with the approved plan. I have included an As-
built Plan for your files.
Please note, this Certification only addresses the installation of the standard Title 5 components.
A separate Certification for the FAST system will be provided by the System Operator. The
Homeowner is reminded of their obligation to maintain an Operation and Maintenance
Contract with the System Operator.
