Dear Mr. Gardiner,
As per your request and in accordance with 310 CMR 15.021(3) of the State
Environmental Code-Title 5, an as-built review of the septic system at the subject site
has been conducted. I hereby certify that the Subsurface Sanitary Disposal System
has been installed in substantial compliance with the design plan by Engineering
Works, Inc. and Richard J. Hood PLS, dated 9/20/23. The septic system consists of an
existing 1500 gallon tank, 1000 gallon pump chamber, new distribution box and a 13' x
25' leach field, as shown on the plan.
