The septic system installed at the subject location by
Speakman Construction was installed per the plan and the
allowances indicating compliance with the State Title V
requirements.
The septic design plan as approved by the Yarmouth Health
Department met the requirements of Title V and the Yarmouth
Board of Health Regulations. The installation of components
meets the requirements of the design plan.
A tee filter has been installed in the outlet tee of the septic tank.
Risers were to within 6" of grade and there was an inspection port
installed at the end of the chambers.
This certification shall not act as a guarantee that the system will
operate as designed as such is dependent on the type of use and
what is disposed in the system by the owner.
