In February 2015, the Commission issued an Order of Conditions for the above -referenced
site, for the construction of a barn, pool, acid other site work.
A site visit was made to review site conditions relative to the Order. The proposed work was
found to be constructed in substantial compliance with the approved plan and Order of
Conditions.
Some previous disturbance was noted West of the stone wal l which may be located on Town
of Yarmouth Conservation land. This area is currently vegetated with fescues and looks to be
reverting hack to a natural state.
This is to certify, to the best of my knowledge and belief, that the work was performed in
substantial compliance with the approved plan and Order of Conditions. We respectfully
request the issuance of a Certificate of Compliance.

B. Submittal Requirements
Requests for Certificates of compliance should be directed to the issuing authority that issued the final
Order of Conditions (OOC). If the project received an OOC from the Conservation Commission, submit
this request to that Commission. If the project was issued a Superseding Order of Conditions or was the
subject of an Adjudicatory Wearing Final Decision, submit this request to the appropriate DEP Regional
