SUBJECT: 25 Lake Road East, West Yarmouth, MA
The septic system installed at the subject location by
B and B Excavation was installed per the plan and the allowances
indicating compliance with the State Title V requirements.
The septic design plan as approved by the Yarmouth Health
Department met the requirements of Title V and the Yarmouth
Board of Health Regulations. The installation of components
meets the requirements of the design plan.
The tank outlet tee filter was installed at the time of the inspection
as required. The water use is not to exceed the design capacity.
This certification shall not act as a guarantee that the system will
operate as designed as such is dependent on the type of use and
what is disposed in the system by the owner.
