I hereby certify that the conditions shown on the plan prepared by Stephen Melesciuc,
Professional Land Surveyor, entitled "Certificate of Compliance As -Built PIan", dated
April 15, 2024, reflect the as -built conditions as of the date of the plan and are in
substantial conformance with the Orders Of Conditions recorded in the Barnstable
County Registry of Deeds in Record Book 10002 Page 206 dated 1/5/1996 DEP File No.
SE 83-1345, Record Book 12469 Page 88 dated 8/11/1999 DEP File No. 83-1481, and
Extension Permit Record Book 15070 Page 72 dated 4/22/2002, MassDep File No. SE
83-1481 and also are in substantial conformance with the proposed plans recorded with
the referenced Orders of Conditions and Extension Permit specifically "Site & Seweage
