RE: Safelite Autoglass - Holding Tank

To Whom it May Concern:
The tank installation was constructed and reviewed in accordance with the
construction documents for Safelite Auto Glass, 40 Whites Path, South Yarmouth,
MA 02664. The plans were designed in accordance with 314 CMR 18. Sheet P102
& P202 reference the tank and were dated 4/5/24 and stamped by Kevin Finlay of
Conway & Owen.
The high level and leak detection alarms have been tested and I certify they function
as intended. The tank was filled with water and the water level held overnight at the
high limit. I certify the tank has been installed and does not leak.
The tank alarm panel indicators are in the process of having permanent labels
affixed.
