The project involves a proposed addition to the eastern face of the dwelling and an expanded deck on the southern face of the dwelling. This application is being submitted in accordance with the Massachusetts Wetlands Protection Act and the Town of Yarmouth Wetlands Protection By-law and associated Regulations. The filing deadline is two weeks prior to the next scheduled Conservation Commission meeting for new applications. Failure to follow this Checklist shall result in an Administratively Incomplete Application and will not be advertised for a Public Hearing. Refer to the meeting schedule. Submitting an Application: Hardcopy filing dropped off or mailed to the Conservation Office including: 1 RDA Administrative Checklist 1 complete RDA application WPA form 1 with original signatures (double sided). * Detailed narrative of the project with sufficient information to enable the Commission to determine whether the proposed work will alter an area subject to protection. If BVW or vegetated wetlands are present, delineation sheet(s), coastal bank calculation and other relevant material if applicable (not applicable for flood zone only projects). Yarmouth Assessor’s locator map highlighting parcel(s) where work is proposed, including map and parcel numbers 1 current abutters list identifying the property owners who are to be notified and abutter notification form. All abutters to be notified via certified mail, Certified Mail Receipts (PS Form 3800) must be provided. 1 original and 6 photocopies of the plan/sketch, folded separately, right side out with title and address visible. All plans shall reference NAVD1988 unless otherwise noted. Landscape plans shall be detailed to show proposed and existing conditions, native species, quantity, size and spacing. The area where the addition and expanded deck are proposed is currently occupied by landscaping, a walkway, and structures associated with the dwelling. b. Request for Determination of Applicability 71 Acres Avenue West Yarmouth, MA PROJECT DESCRIPTION 1.0 INTRODUCTION The project located at 71 Acres Avenue, (the Site) in West Yarmouth involves a proposed addition to the eastern face of the dwelling and an expanded deck on the southern face of the dwelling. This application is submitted in accordance with the Massachusetts Wetlands Protection Act and the Town of Yarmouth Wetlands Protection By-law. 2.0 SITE DESCRIPTION The project site consists of single-family residential property bordered by other single- family dwellings. The wetland resource areas and areas of conservation jurisdiction on the Site that are protected under the Wetlands Protection Act and the Town of Yarmouth's Wetland Protection Bylaw include Land Subject to Coastal Storm Flowage (LSCSF) (Zone AE, elevation 11). The Site is outside of all other jurisdictional resource areas and their associated buffer zones. 3.0 PROJECT ACTIVITIES The project involves a 10' x 26' addition to the eastern face of the dwelling which faces the roadway and the expansion of an existing deck on the southern face of the dwelling. These structures will be attached, and stairways/entrance steps will also be constructed, respectively. The area where the addition and expanded deck are proposed is currently occupied by landscaping, a walkway, and structures associated with the dwelling. Excavation will be limited to the footprint of these structures and no areas of natural vegetation will be removed in association with this work. The proposed construction schedule will likely follow typical procedures for additions to dwellings. Access will be gained from Acres Avenue and the gravel driveway. Sediment controls will be installed at the limits of work around the Site and roll -off containers will be positioned in driveway/lawn areas which will be used to temporarily store construction debris. Various construction machinery which may include a skid steer, backhoe, etc will be employed to demolish the various features/structures on the site which cannot be demolished/removed by hand. Following demolition work, the addition and deck replacement work will occur, followed by various restoration and landscaping activities. Any rutting will be buffed and/or filled with clean fill, loamed, and seeded with grass seed. Once the site reaches appropriate levels of stabilization, the sediment controls will be removed from the Site. 4.0 MITIGATION The proposed project has been designed to reduce impacts to jurisdictional resource areas to the greatest extent practicable. A line of staked straw wattles and/or silt fence will be installed at the limit of work prior to the start of project activities. These controls will remain in place and in operational condition until final site stabilization is complete. All construction materials will be staged as far from resource areas as possible. All debris from the removal of various structures, landscaping components, and various construction materials will be contained within an appropriate receptacle (roll-off containers or similar) and/or immediately removed from the Site. Although the proposed work will occupy addition area within LSCSF, the structures will be attached to an existing dwelling and should not result in significant displacement of stormwater which might result in erosion or direct additional stormwater towards neighboring properties.
