On behalf of my client Jason Lilly, please accept this request for a Certificate of Compliance for the above
referenced project. A site inspection was performed by Shorefront Consulting on July 17, 2024, with the
results communicated to Existing Grade, the P.E. for the project. The proposed dock reconstruction and stair
reconstruction work is complete, and the plantings have been installed and are stabilized. The buffer strip was
measured and found to be in compliance with the order of conditions, special condition #16. The float stops
were observed and photographed. An as-built plan was not required as part of the order. In addition, the
bulkhead work was not performed. The order has since expired and the applicant would like to close it out.
Photographs are included in this request. The project was monitored and inspected by Shorefront Consulting
during the 2013 construction process. The dock and stairs were re-constructed according to the permit, with no
modifications or deviations noted. There are no deviations from the plan observed during the time of
inspection.
Therefore, the project has been completed in accordance with the Order of Conditions, and all Special Conditions
contained in SE-83-1961 issued on 4/2/2013. Attached please find Yarmouth form WPA-8A Request for
Certificate of Compliance, photos of the completed project, and a check for $75 made payable to the Town of
Yarmouth. The work results have been reviewed by Existing Grade, and below is their certification of this request
letter.
Please include this request in your next public hearing. The original Certificate of Compliance should be mailed
to Shorefront Consulting for recording. If there are any questions or concerns regarding this request, please

B. Submittal Requirements
Requests for Certificates of Compliance should be directed to the issuing authority that issued the final
Order of Conditions (OOC). If the project received an OOC from the Conservation Commission, submit
this request to that Commission. If the project was issued a Superseding Order of Conditions or was the
subject of an Adjudicatory Hearing Final Decision, submit this request to the appropriate DEP Regional
