All filings must be made on Town of Yarmouth forms.
All applications must be submitted two weeks prior to the next scheduled Conservation
Commission meeting. Refer to Meeting Schedule.
Failure to follow the Administrative Checklist shall result in an Administrative Incomplete
Application and will not be scheduled for a Public Hearing.

B. Submittal Requirements
Requests for Certificates of Compliance should be directed to the issuing authority that issued the final
Order of Conditions (OOC). If the project received an OOC from the Conservation Commission, submit
this request to that Commission. If the project was issued a Superseding Order of Conditions or was the
subject of an Adjudicatory Hearing Final Decision, submit this request to the appropriate DEP Regional

I have inspected the pier repairs at your building at 51 South Shore Drive in South,
Yarmouth. At this time, the work has been completed in substantial compliance with the.
design plan. The clips that have been added to connect the old framing to the new
construction are adequate for their intended use.

The Order of Conditions from the Conservation Commission for this project is valid for
three years. You can request a Certificate of Compliance at this time, or simply leave the
Order of Conditions in effect in the event that further repairs may be necessary in the
future.
I also reviewed the driveway construction under way. The construction is more substantial
than I had initially anticipated --timbers have been installed along both sides of the
driveway in order to raise the driveway grade as you leave the street and approach the
door. While this will make an attractive driveway, it will most likely increase flooding in
the road area. I also anticipate that the narrow area between the edge of the driveway and
the adjacent building will see accelerated erosion from waters draining from the street to
