The filing deadline is two weeks prior to the next scheduled Conservation Commission meeting for new applications. Failure to follow this Checklist shall result in an Administratively Incomplete Application and will not be advertised for a Public Hearing. Refer to the meeting schedule. Typed signatures will not be accepted. ✓ Detailed narrative of the project including existing & proposed conditions, construction sequence, type of equipment, staging locations, drainage and stormwater, erosion controls, invasive species management and alternative analysis. The narrative shall include how the project meets performance standards per 310 CMR 10.0 & TOY Wetland Regulations, delineation sheets (if BVWor vegetated wetlands are present) or other resource area calculations, and supporting information. ✓ 1 100' radius map, current abutters list identifying the property owners who are to be notified per 310 CMR 10.00 and abutter notification form. Abutters list must be certified by the Town Assessor's office. The Assessor's office requires 7 days advance notice. All abutters must be notified via certified mail. ✓ Certified Mail Receipts (PS Form 3800) for all abutters. Green cards to be submitted at the hearing. ✓ 1 original and 6 copies of the plan, folded separately, right side out with title and project address visible. All plans shall reference NAVD1988 unless otherwise noted. A front porch and rear deck will be added along with the required additional steps down to grade. The 7,000 s.f. property is located in a residential neighborhood and within Land Subject to Coastal Storm Flowage (LSCSF) flood zone AE (EL 11 ). The property is improved with two dwellings and the applicant is proposing to leave the rear structure as is. The new front dwelling will be designed to meet current flood zone regulations and will continue to utilize the existing septic system. Drywells or gravel drip edges are proposed to infiltrate roof runoff into the sandy subsoils. No other wetland resource areas are within 100' of the site. An Order of Conditions was issued 1/15/2021 for a similar scope of work (SE83-2260). A request to close out that Order will be subsequently filed pending results of this new filing. No work will take place within Priority/Estimated Habitat of endangered species. Exposed soils will be loamed and seeded within 30 days of completion of the work and final grading work as weather permits. Land Subject to Coastal Storm F/owage The site is affected by 100-year flood elevation AE (EL 11). The proposed dwelling will conform to flood zone and building code standards. No fill is proposed so construction and grading activities will have no affect on the 100 year flood elevation. Compliance with the MA Wetlands Protection Act and Yarmouth Bylaw Chapter 143 Regulations: The proposed construction activities will not have a deleterious effect upon any wetland values as listed in the Bylaw. Roof drainage will be mitigated utilizing downspouts to drywells or drip lines to stone trenches. The new dwelling will be brought up to flood zone compliance. Construction protocol All machinery will access the areas of proposed construction through the existing driveway. The front structure will be raised up on temporary blocks, the foundation hole will be excavated, the foundation poured, and final construction will begin.
