Dear Mr. Silva.
We have reviewed the records ofthe Yarmouth Health Department and have identified you as the owner
of the properties above.
ln accordance with Town of Yarmouth General Code, Chapter 108 - Occupancy of Buildings, it is
mandatory for all rental properties to be registered with the Yarmouth Health Depafiment annually. The
registration fee for each unit is S80 per year.
To ensure compliance, we kindly request that you complete and sign the enclosed registration, providing
all the required information, within 7 days ol receiving this letter. Alternatively, you can conveniently
register online by visiting our website at https://www.yarmouth.ma.us/ 127lHealth and following the
rental registration instructions.
Failure to comply with the registration requirements, as stated in $108-2 ofthe Town of Yarmouth
Brlau, can result in a fine of up to $300 per day, with each da)' constitutes a separate offense.
