A site inspection was performed by Shorefront Consulting on October 1, 2024 to observe and photograph additional plantings along the retaining wall. The proposed work is complete. An As-built plan and photographs are included in this request. The as-built plan was not required as part of the order closeout conditions, but is included for information only. All measurements taken to locate the structures were taken by Shorefront Consulting in the field and transferred to create the as-built plan, which is not stamped. There is an additional section of fencing that is outside of the commission’s jurisdiction on the north side of the property, which was not specifically located in the field. There was a revised plan (revision #2) dated 6/6/2016 to move the access stairs and dock over 5 feet to the west to comply with the Chapter 91 License 25-foot setback requirement. That plan was submitted and is on file with the town’s documents. The project was monitored and inspected by Shorefront Consulting during the construction process. The following deviations from the permit plan are noted below: 1) The seasonal dock was never installed. 2) The proposed 15’x27’ pervious patio was not installed. Therefore, the project has been completed in accordance with the Order of Conditions, and all Special Conditions contained in SE-83-2044 issued on 1/19/2016. Attached please find Yarmouth form WPA-8A Request for Certificate of Compliance, photos of the completed project, an as-built plan dated 10/14/24, and a check for $75 made payable to the Town of Yarmouth. Please include this request in your next public hearing. The original Certificate of Compliance should be mailed to Shorefront Consulting for recording.
