This department is in receipt of a subsurface sewage disposal system inspection report conducted by Kevin Bloomuist, regarding the above referenced address. The inspector conditionally
passed the system for the following reason(s):
The D box was in failure
As the owner of the property, you have sixty (60) days from the date of inspection to begin the process of bringing the septic system up to State Title 5 regulation requirements.
