B. Submittal Requirements
Requests for Certificates of Compliance should be directed to the issuing authority that issued the final
Order of Conditions (OOC). If the project received an OOC from the Conservation Commission, submit
this request to that Commission. If the project was issued a Superseding Order of Conditions or was the
subject of an Adjudicatory Hearing Final Decision, submit this request to the appropriate DEP Regional

In October 2004, the Commission issued an Order of Conditions for the above-referenced site,
for the reconstruction and repairs to an existing bulkhead wall.
A site visit was made and a survey of the existing conditions relative to the Order was carried
out. The proposed vinyl s~eet pile wall appears to have been constructed in compliance with
the approved plans.
This is to ce1tify, to the best of my knowledge and belief, that the work was perfo1med in
substantial compliance with the approved plan and Order of Conditions.
We respectfully request the issuance of a Certificate of Compliance.
