SCOPE OF SERVICES:
Tighe & Bond, Inc. will perform the services outlined in Attachment 1 regarding the Operation and Maintenance of the Wastewater Treatment System at the above-noted location.

ATTACHMENT 1
OPERATION AND MAINTENANCE
SCOPE OF SERVICES
The following is a summary of the scope of services to be provided by CONSULTANT:
The treatment system shall be operated by a Certified Wastewater Plant Operator in accordance
with the requirements of 257 CMR 2.00 and the Board of Certification of Operators of Wastewater
Treatment Plants. The treatment system shall also be operated in accordance with the conditions established by the Massachusetts Department of Environmental Protection (MassDEP) under 310 CMR 15.000 Title 5 of the Massachusetts Environmental Code for the permitted use and with the
local Board of Health.
EQUIPMENT MAINTENANCE
1.Within design capacity and capability of the equipment, maintain the wastewater system forthe benefit of CLIENT.2.Certify and document all maintenance for the system. Maintenance reports will be provided by
request of the CLIENT.
3.Certify and document all repairs to the equipment.
4.Facilitate emergency repairs in the most expeditious and cost-effective manner.
5.Perform other services that are incidental to the services specified here including facilitating
emergency repairs in the most expeditious and cost-effective manner at an additional cost asrequested by CLIENT.6.Pump maintenance to be performed in accordance with manufacturer’s specifications by
subcontractor and invoiced by them directly to the CLIENT.
7.Inspect septic tanks.
BIOCLERE SYSTEM MAINTENANCE
1. Standard maintenance as follows:a.Check general condition/appearance of unit.b.Check vent flow, odor.
c.Check general condition of fan box including internal and external wiring, lock, latch, gaskets, etc.
d.Check quiet fan operation.
e.Check condition of cover locks, latches, gaskets.
f.Check and characterize biomass.
g.Check recycle pump operation, timing and effluent clarity.
h.Check dosing pumps operation, timing, effluent clarity and spray pattern.
i.Check general condition of dosing assembly. Clean nozzles if required.j.Check general condition of control box including locks, gaskets, etc.k.Check control box switches, alarms, timers, etc.l.Complete and maintain service report file.m.Check septic tank quarterly.2.Maintenance frequency as follows: monthly Operation and Maintenance visits to perform standardBioclere maintenance.
BIOCLERE SYSTEM SAMPLING
In accordance with the Department of Environmental Protection Standard Conditions for Secondary
Treatment Units Approved for General Use, sampling and monitoring will be conducted as follows: 1.Collect quarterly effluent sample and field test for pH and color.
2.Collect quarterly effluent sample and test for Nitrate, TN, Ammonia, Alkalinity, Nitrite, TKN,
CBOD5, TSS at a certified lab.3.On an annual basis measure:
•Thickness of floating grease and sludge/scum in the septic tanks and other processtanks.
Reporting: Prepare summary reports following each operation and maintenance visit and sampling event. Reports will include field reports and test results and will be submitted to the Barnstable County
Department of Health and Environment, and the local Board of Health.
Notes:
1.CONSULTANT will not perform any procedures which will require confined space
entry.
2.Services under this contract specifically do not include or cover any responsibility forsystem malfunction attributed to process design, equipment specified and/orinstallations as provided by others.3.CLIENT must provide access to all system components at time of the O&M visit.
4.This service contract assumes year-round occupancy of the dwelling or facility. The
CLIENT shall notify CONSULTANT if occupancy becomes seasonal.
5.CONSULTANT will notify the CLIENT and the appropriate authority of any event ofelectrical or mechanical failure observed by CONSULTANT within the treatment system,or of any event observed by CONSULTANT which may adversely affect the performance of
the treatment system.6.In the event that the system alarm is activated, and the system fails, the CLIENTshall notify CONSULTANT in a timely manner to allow CONSULTANT to notifyMassDEP and Board of Health within 24 hours of the alarm system activation.
Corrective action shall be taken upon notification and will be treated as additional
services. CONSULTANT shall not be liable for any damages or fines should the
CLIENT delay notifying the CONSULTANT as noted above.
