To Mr. Wilson:
The Town of Yarmouth Health Department is ordering a septic inspection to be
conducted at the barn ,on the cranberry bog, with the shower and washer & dryer at 16 Denver drive in West Yarmouth, MA.
15.301: System Inspection
“(9) All systems shall be inspected when the owner or operator thereof is ordered to
do so by the local Approving Authority, the Department or court.”
Therefore, you must have your Septic System inspected by a certified Title 5
Inspector within 30 days of receipt of this letter. Failure to comply may result in further Board of Health Action.
You have the right to appeal the above orders by requesting a hearing before the Board of Health. Said request shall be in writing and received in the office of the
Health Department within 7 days of receipt of this notice. At said hearing; you have the right to be heard and to present witness and documentary evidence as to why this
order should be modified or withdrawn. You have the right to be represented by an attorney and to inspect and obtain all relevant inspection or investigation reports,
orders, notices, and other documents in possession of this office. Also, any adverse party has the right to be present at said hearing.
