RE: Subsurface Sewage Disposal System Inspection Report

Dear Homeowner:
This department is in receipt of a subsurface sewage disposal system inspection report conducted by Chad hathaway, regarding the above referenced address. The inspector failed the septic
system for the following reason(s):
The SAS was in hydraulic failure at the time of inspection. System full of roots and pushing blocks into itself. Also the dwelling is in a zone II with 2 bedroom max. A third bedroom
was added. One bedroom must be removed. When garage was added many notes say not to be used as a bedroom in the folder.
As the owner of the property, you have sixty (60) days from the date of inspection to begin the process of bringing the septic system up to State Title 5 regulation requirements.
