Dear Commission Members: On April 9, 2001, the Commission issued an Order of Conditions for the above -referenced site, for a proposed dwelling and Title 5 septic system. A site visit was made in November of this year. The septic system was installed as per the design plan, however the guest house was never constructed. Please note that subsequent alterations to the site were approved with an updated plan and & Order of Conditions (SE83-1894), with a request for a Certificate of Compliance imminent. To the best of my knowledge and belief, the septic system and dwelling were completed in substantial compliance with the approved plan and Order of Conditions.
