This department is in receipt of a subsurface sewage disposal system inspection report conducted by Michael Sears, regarding the above referenced address. The inspector failed the septic
system for the following reason(s):
The SAS was in hydraulic failure at the time of inspection. Tank shows signs of overloading, D box outlet line root bound, Entire area of system was overgrown with vegetation and roots
As the owner of the property, you have sixty (60) days from the date of inspection to begin the process of bringing the septic system up to State Title 5 regulation requirements.
