Called to 1 1/2 story wood frame type 5 building with brown smoke showing from the attic area. Upon arrival E-42
investigated found fire in 2nd floor bathroom exhaust Fan. Attempted to extinguish with pressurized water container. Fire
extended to attic area. 1 3/4 hose line was put in place along with an attic ladder to access scuttle hole. C-58 took command
calling for working fire assignment due to limited resources because of multiple calls. C-58 confirmed all residents out of
house. Dennis 111, E-45, E-42 dispatched on initial call. Outside companies from Hyannis, Brewster, Barnstable called in to
cover stations. E-45 on scene secured water supply for E-42 and helped with roof operations. E-111 Assigned RIT team and
assisted with setting up PPV at front door. DFD Car 3 assigned side C. Fire knocked down extensive overhaul due to blown in
insulation. Car 51 on location. E-45 ventilated roof by using vent pipes and exhaust covers. Utilities secured both electrical
and gas. Wiring inspector called and on location. Command terminated OPS channel released. Investigation done and written
in additional narrative space.

Additional Narrative (i2 of 2):
Called for fire in the bathroom extension to the attic. Investigation verified this information. Talking with homeowner he
stated he had vent fan in bathroom running for approximately 10 min before hearing a screeching noise when he went to
investigate he found fire and melting plastic dripping from fan. He took his child and left. Investigation shows Nutone Fan
Model # 668RP with heavy fire damage and v pattern in the area of the motor and plastic fan assembly. Wiring inspector on
location confirms he believes fire started not initially in the wiring but the mechanical motor of the fan. No fuses tripped in
the fuse box FD shut of main breaker on fuse box upon arrival. Homeowner advised they cant stay in house due to water,
smoke and fire damage. They were collecting belonging s and making arrangements to leave. Pictures were taken of the
motor, fire origin and electrical panel. Homeowner advised to call insurance company and a restoration company to close the
roof and help with water evacuation.

record that unfortunately due to a technical
error on the conference call, I was off the
conference call for roughly 15 minutes.
During one of the breaks, as we returned, I
came back on so there was some questioning
that I missed. Until I see the transcript,
I am going to refrain from closing my
portion of the questioning. I doubt I'll
have follow-up but I don't know because I
don't know what was asked. And there's no
