scheduled Conservation Commission meeting for new applications. Failure to follow this Checklist shall result in
an Administratively Incomplete Application and will not be advertised for a Public Hearing. Refer to the meeting

Property Description
The property is a 21,037 SF developed single family site. The property is located
within Land Subject to Coastal Storm flowage (Zone AE, elev. = 11'; NAVD88).
The existing ground elevations were established by a topographic survey using a
NAVD 88 bench mark. The existing first floor elevation = 11.22'.
The Site is developed with a three -bedroom single-family dwelling, deck, patio,
and shed. According to Assessor's records, the dwelling was constructed in
1975. The existing Title 5 septic system is located in the front yard and consists
of a 1500-gallon septic tank and leaching field. Hardscaping includes a paved
circular driveway and brick walkway. Crushed stone surrounds the deck and
foundation.
Project Description
The Applicant proposes to construct an addition (20.5' x 16') on the back of the
building. The addition will be constructed on posts supported by sonotubes (no
cellar). This will allow flood waters to flow unimpeded around the existing
foundation. The dwelling will remain three bedrooms
The proposed work will occur within existing lawn and stone area and partially
within the footprint of the existing deck. Upon completion of the work, all
disturbed areas will be loamed and seeded with a drought -tolerant native grass
mix. ("Harmony" low maintenance grass mix by Colonial Seed or approved
equal)
Construction Protocol
All construction vehicles shall remain in the front yard. The existing septic
system components shall be adequately protected to prevent damage by
construction equipment.
A'mini-excavator or similar equipment shall excavate soil for the installation of
the proposed sonotubes. If necessary, a steel plate shall be placed over the
system components to provide access to the back yard.
A dumpster shall be placed in the driveway and it shall be provided with a
suitable lid or cover to prevent wind-blown debris.
All demolition debris shall be placed in dumpster or directly into trucks for
removal from the site and proper disposal.
Crushed stone at the roof dripline shall be installed as necessary to contain roof
runoff from the proposed addition.
Upon completion of construction, all disturbed areas shall be loamed and seeded
with a native grass mix ("Harmony" grass seed mixture by Colonial Seed or
approved equal).
Throughout construction, all debris shall be placed in a dumpster or other
suitable container. The container shall be provided with a suitable lid or cover to
prevent wind-blown debris. The site shall be cleaned of debris on a daily basis.

In accordance with the second paragraph of the Massachusetts General Laws Chapter 131, Section 40,
you are hereby notified of the following:
A. The name of the applicant is David C. & Marie K M Melaragni
B. The applicant has filed a Request for Determination of Applicability ("RDA") with the Yarmouth
Conservation Commission, seeking permission to remove, fill, dredge or alter an Area Subject to
Protection under the Wetlands Protection Act (MGL c. 131 s. 40 & Town of Yarmouth Wetland By -Law,
Chapter 143).
C. The address of the lot where the activity is proposed is 1 Seaside Village Road
D. Proposed work is an addition to the existing dwelling which shall be constructed on posts
supported by sonotubes. The work will occur within land subject to Coastal Storm Flowage.
