I, Kathryn D. Chouinard, of 8 Chatfield Terrace, Enfield, CT 06082, on
oath, say and depose as follows:
1. That as trustee of the Kathryn D. Chouinard Revocable
Trust Indenture dated March 6, 2000 (and recorded in the Barnstable County
Registry of Deeds Book 15190, Page 138), I am the owner of the real estate
located at 56-56A Seaview Avenue, South Yarmouth, Ma.
2. That this property was acquired as vacant land by my parents, Richard A.
Dunn and Kathryn P. Dunn, in 1957 (see deed recorded in the Barnstable County
Registry of Deeds in Book 986, Page 190; and in the 1960's the dwellings that
exist on the land were constructed by my father who was a builder. Since the
construction of these dwellings, this property has been used continuously as a two-
family, residential dwelling.
The foregoing is based upon personal knowledge and information.
