Dear Mr. Amado,
We have reviewed the records of the Yarmouth Health Department and have identified you as the
owner of the property above.
In accordance with Town of Yarmouth Bylaw, Chapter 108 - Occupancy of Buildings, it is
mandatoryfor all rental properties to be registered with the Yarmouth Health Department annually.
The registration fee for each unit is $80 per year.
To ensure compliance, we kindly request that you complete and sign the enclosed registration,
providing all the required information, within 7 days of receiving this letter. Alternatively, you can
conveniently register online by visiting our website at https://www.yarmouth.ma.us/127/Health
and following the rental registration instructions.
Failure to comply with the registration requirements, as stated in §108-2 of the Town of
Yarmouth Bylaw, can result in a fine of up to $300 per day, with each day constitutes a
separate offense.
If you are NOT renting for 2024, please fill out and send back the enclosed affidavit.
