Ojala, P.E., P.L.S. Arne H. Ojala P.E., P.L.S. Daniel E. Gonsalves, P.E., S.E. Craig J. Ferrari, E.I.T., S.E. An Order of Conditions was issued for 9 Flakeyard Lane in West Yarmouth for a raze and replace of an existing garage, SE83-2138. A site visit was made in April of 2025 and an as-built plan and photographs representative of the site are attached. The work was found to be in general conf01mance with the approved plans with some minor deviations. These changes include wooden walkways on either side of the garage, a rinse station, and a smaller gravel driveway. These changes result in a net increase of 65 s.f. of hardscape areas from the approved plan. Gravel drip edges were installed for the roof runoff and the driveway is a pervious gravel material. This is to ce1iify, to the best of my knowledge and belief, that the work was performed in substantial compliance with the approved plan and Order of Conditions.
