The filing deadline is two weeks prior to the next scheduled Conservation Commission meeting for new applications. Failure to follow this Checklist shall result in an Administratively Incomplete Application and will not be advertised for a Public Hearing. Refer to the meeting schedule. Typed signatures will not be accepted. ✓ Detailed narrative of the project including existing & proposed conditions, construction sequence, type of equipment, staging locations, drainage and stormwater, erosion controls, invasive species management and alternative analysis. The narrative shall include how the project meets performance standards per 310 CMR 10.0 & TOY Wetland Regulations, delineation sheets (if BVW or vegetated wetlands are present) or other resource area calculations, and supporting information. ✓ 1 100' radius map, current abutters list identifying the property owners who are to be notified per 310 CMR 10.00 and abutter notification form. Abutters list must be certified by the Town Assessor's office. The Assessor's office requires 7 days advance notice. All abutters must be notified via certified mail. ✓ Certified Mail Receipts (PS Form 3800) for all abutters. Green cards to be submitted at the hearing. ✓ 1 original and 6 copies of the plan, folded separately, right side out with title and project address visible. All plans shall reference NAVD1988 unless otherwise noted. The site consists of a developed approximately 28,606 s.f. lot improved with a 4 bedroom single family dwelling. The site is bordered by tidal Salt Marsh to the East, the resource area was flagged by Brad Hall of BLH Environmental Consulting on January 28, 2025. The site is also within Land Subject to Coastal Storm Flowage (LSCSF) Flood Zone AE elevation 14, VE elevation 16 and Zone X. There is a Coastal Bank coincident with the Flood Zone which bisects the existing dwelling. All work will occur outside the 200' riparian zone to White's Brook. The site does not lie within Priority or Estimated Habitat in accordance with the latest Natural Heritage map (2017). The addition will consist of an open porch and a small entryway enclosure. It will be located above the Coastal Bank and outside LSCSF. The porch will be 14' away from the Coastal Bank at its closest point. And the closest point ofthe·new addition will be 57.9' from the Salt Marsh. Mitigation plantings are proposed totaling 800 s.f. comprised of native shrub species located within the existing lawn area adjacent to the Salt Marsh in the rear of the property. A work limit line of staked silt fence is proposed as shown on the plan. Salt Marsh (310 CMR 10.32) The edge of the salt marsh was flagged by Brad Hall of BLH Environmental Consulting and located by Down Cape Engineering. The closest part of the addition will be 57.9' to the edge of the salt marsh. All work will occur within existing lawn areas. No adverse effects from the work are expected, as the work limit line shall remain staked in place until all work is complete, and the site is stabilized. The salt marsh will continue to act fully in its capacity to provide habitat and food sources for wildlife. Land Subject to Coastal Storm Flowage The property is within Floodzones AE el. 14, VE el. 16 and Zone X. The proposed addition work will occur within flood zone X (area of minimal flood hazard). Only the proposed mitigation plantings are to occur within the AE el. 14 flood .zone. No fill is proposed so construction activities will have no effect on the 100 year flood elevation. Coastal Bank A Coastal Bank exists at the 100 yearfloo,d plain on the moderate slope to the South of the existing house. The Coastal Bank runs up slightly to the top of the steeper slope at the North of the structure. The Coastal Bank was delineated in accordance with the Wetlands Program Policy 92-1. The bank's ability to control floodwaters will not be changed by any of the proposed work.
